Position Description

Summit Home Care, a leader in the home care industry is seeking a Nurse Administrator.

The Nurse Administrator will be in the office 50% of the time and in the field 50% of the time.

The Nurse Administrator provides assistance to individuals in a manner that takes into account their special needs and age-related needs. They provide supervision and direction for field nursing staff and act as a liaison between field and office staff. They would evaluate and address customer concerns, including documentation and resolution of the same. Monitors patient's clinical records concurrently and retrospectively, utilizing agency key performance metrics/ indicators. They are responsible for communication to physicians/ obtaining orders for delay in admissions visit or orders related to care. Collaborates with Intake on acceptance of referrals to team. Communicates with referral sources for issues that may arise. They conduct regular case conferences with clinicians evaluating for appropriate utilization and care planning. Audit clinical records for timely and accurate documentation and follows up as necessary. Responsible for monitoring HCHB reports related to team performance. Mentors and coaches staff to improve enhance performance as needed. Participate in the interview and hiring process for new team members.

Job Requirements:
A graduate of accredited school of nursing, BSN preferred
Current New York State RN license
Minimum five years nursing experience
Previous experience with HHA Exchange a plus
Must be comfortable with technology
Management experience
